Here is a friendly reminder of some things YOU MUST DO prior to your appointment (Please read and only do what applies to you):
1. For NEW CLIENTS ONLY OR IF YOU HAVEN’T FILLED THIS FORM OUT BEFORE OR IF YOU ARE HAVING A NEW SERVICE DONE WITH US - If you are having any Eyelash or Eyebrow service done please follow these instructions: Click or copy and paste this link into a web search bar. Fill out the form at least 24 hours before your appointment: https://form.jotform.com/213628576831059 You are only to fill out this form if you are a first-time/new client and have never been to Hannah Davis Lashes before. This form is only to be filled out ONE TIME! You can also find this form at hannahdavislashes.com under the "Client Forms" tab.
2. If you are having Cosmetic Teeth Whitening done please fill out this form: https://form.jotform.com/220537562270048 (Copy and paste the link into a web search bar, fill it out, and submit it.) - This must be done each time you come in for this service, it does not matter if you are a recurring client and have filled out this form before. You can also find this form at hannahdavislashes.com under the "Client Forms" tab.
3. If you are having a Tooth Gem or Gems applied please fill out this form: https://form.jotform.com/220540873549056 (Copy and paste the link into a web search bar, fill it out, and submit it.) - This must be done each time you come in for this service, it does not matter if you are a recurring client and have filled out this form before. You can also find this form at hannahdavislashes.com under the "Client Forms" tab.
4. Masks are optional, you can bring and wear one if you'd like but they are no longer required.
5. Please come with a clean/natural face (no makeup, especially on the eyelashes and/or eyebrows) ONLY APPLIES FOR EYELASH & EYEBROW SERVICES.
PLEASE TAKE THE TIME TO READ OUR FOLLOWING POLICIES:
- All clients must cancel their appointment 48 hours prior or the card provided when booking will be charged a $20.00 cancellation fee. If you no-show your appointment, meaning you fail to show up without letting us know, the full amount of the service will be charged to the card you provided when booking and you will no longer be allowed to book an appointment with us.
- You will be sent a text and email 3 days prior to your appointment asking you to confirm your appointment slot. All appointments must be confirmed no later than 24 hours before the appointment, if an appointment is not confirmed by the 24 hour mark the appointment will be cancelled and the client will be subject to pay a cancellation fee charge.
- If your cancellations become a recurring pattern (meaning 2-3 times in a row) we may decline you as a client in the future.
- We accept Cash and Card payments. Contactless payments (Apple pay, Google pay, contactless card payments) and Chip payments. Visa, Mastercard, American Express, and International credit cards.
- Please bring exact change if you are paying cash, we do not always have change on hand.
- If you are paying with anything but cash, a tax of (13%) will be added to the total cost. If you are paying cash no tax will be added.
- APPLIES TO LASH EXTENSION SERVICES: Your eyelashes will be cleaned complimentary prior to your fill or full set however, If you arrive with any makeup or any kind of buildup/debris in the eyelashes that requires extra time and products to clean you will be charged an extra $10.00 automatically.
- APPLIES TO LASH SERVICES: Avoid any oils/oil-based products on your face/around your eyes prior to your appointment.
- APPLIES TO TEETH SERVICES: Please brush your teeth before your appointment.
- Please arrive on time for your appointment as there are appointments before and after you. If you are 15+ minutes late your appointment will be cancelled and you will be charged the full service amount you were booked in for on the card you provided when booking.
- If you are coming for a fill your lashes must be 40% full, if they are not you will be presented with a few different options at your appointment.
- Upon arrival please do not park in the driveway and text before ringing the doorbell/knocking on the door. Text/Dm Hannah and let her know you have arrived from your car and you will be instructed as to when it is okay to come to the door.
- No guests are permitted.
